Skilled Care Services

Skilled care refers to clinical medical services delivered by licensed professionals such as registered nurses, physical therapists, occupational therapists and speech therapists. These services typically require a physician’s order and may include dressing changes, wound care, medication administration, IV therapy, and/or education while recovering from a recent hospitalization or surgery.

Catastrophic Care

We provide comprehensive, long-term care for individuals facing life-changing, catastrophic injuries such as spinal cord damage, traumatic brain injury, or severe illness. Services include 24/7 caregiving, medication administration, ventilator and tracheostomy care, bowel and bladder management, mobility support, and frequent nursing oversight to ensure health and safety. We work closely with families, physicians, and case managers to ensure every detail of care is addressed.

Workers' Compensation Care

PHS supports workers recovering from job-related injuries with in-home care under workers’ compensation. Our nurses and caregivers provide daily documentation — ensuring compliance with medical recommendations and coordination with case managers and insurance carriers to ensure 
a fast recovery.

IV Services

Our licensed nurses administer in-home infusion therapy, including hydration, antibiotics, and nutritional support. We adhere to best practices for aseptic technique, central line care, and patient education to deliver high-quality IV treatment from the comfort of home.

Advanced Wound Care

Preventative Care Management

This service helps prevent hospital readmissions and supports chronic condition management through regular check-ins, vital sign monitoring, medication reconciliation, and client education. Our goal is to reduce complications while promoting long-term wellness.

Non-Skilled Care Services

Non-skilled care, also known as personal care or custodial care, focuses on activities of daily living support – helping with tasks such as transferring, toileting, bathing, dressing, eating, and continence. These services are often provided by caregivers and aides and do not require a physician’s orders.

Veteran Support

Long-Term Care Insurance Navigation

Medicaid-Certified Programs

PHS is a contracted Medicaid provider participating in three non-skilled programs:
  • Personal Care Services: Assistance with bathing, 
grooming, dressing, meal prep, medication reminders, 
and light housekeeping.
  • Frail Elderly Care Services: Support tailored for seniors needing help with daily living activities to remain safely at home. Our goal is to preserve independence and enhance quality of life for both the client and their family.
  • Physical Disabilities Care Services: Personalized care for individuals with disabilities, including mobility support, safety oversight, and routine assistance. We assist families with eligibility, care plan development, and ongoing 
case coordination.
