What to Expect from the PHS Process

Choosing a provider is a significant decision, and we’re here to make each step smooth, clear, and supportive.
Initial Inquiry
Your journey with PHS typically begins with a phone call or online inquiry. During this first interaction, our team learns about your needs, answers your initial questions, and discusses the types of care we provide prior to scheduling an evaluation.
Initial Evaluation
Next, we schedule an in-home evaluation by a licensed nurse or care professional. This allows us to understand your medical background, daily living environment, personal preferences, and overall care needs. It’s also an opportunity for you to ask questions in a relaxed setting.
Customized Care Plan Development
Based on your assessment, our team creates a personalized care plan tailored to your specific needs. This outlines services, visit frequency, caregiver responsibilities, and short- and long-term goals. We collaborate with your family and any referring physicians or case managers, as needed. We also coordinate with hospitals, social workers, and discharge planners to ensure a seamless transition from facility to home.
Care Team Introduction & Service Start
Once your care plan is approved, we introduce you to your dedicated care team. Services begin promptly and with your preferences top of mind. We strive to match clients with consistent caregivers for comfort and continuity.
Ongoing Communication
Throughout your care, we maintain close communication and make plan adjustments as your needs evolve. Our team provides regular updates and remains available to answer questions or troubleshoot concerns.

Frequently Asked Questions

What services does PHS provide?
Skilled care refers to clinical medical services delivered by licensed professionals such as registered nurses, physical therapists, occupational therapists and speech therapists. These services typically require a physician’s order and may include dressing changes, wound care, medication administration, IV therapy, and/or education while recovering from a recent hospitalization or surgery.
Non-skilled care, also known as personal care or custodial care, focuses on activities of daily living support – helping with tasks such as transferring, toileting, bathing, dressing, eating, and continence. These services are often provided by caregivers and aides and do not require a physician’s orders.
Do you accept Medicare?
No. Coming soon!
Do you accept Medicaid?
Yes. PHS accepts straight Medicaid services for non-skilled only. PHS does not accept any MCO plans. We accept Type 30 Personal Care Services, Type 48 Frail Elderly Care Services, and Type 58 Physical Disabilities Care Services. Our intake team helps you navigate eligibility and paperwork.
Are you contracted with the VA?
Yes. PHS is an authorized VA Community Care Network Provider for non-skilled home care providing ADL services.

In the near future, PHS will provide skilled nursing and therapy services.
Can I use my long-term care policy to pay for services?
Yes. LTC policies will pay for activities of daily living services. With our advanced knowledge of long-term care policies, our team can help guide patients through the claim process with the insurance carrier.
Who provides my care?
Our care teams include licensed nurses, certified nursing assistants, and trained home care aides. All caregivers are background checked, thoroughly trained, and chosen with your needs and comfort in mind.
Will I always have the same caregiver?
That is the goal. We strive to provide continuity and consistency. The best way to accomplish that goal is to maintain a consistent schedule and to be kind to your caregiver. If a change is ever needed, we will work with you to ensure the new caregiver is a good fit.
Can I adjust my care plan?
Your care plan is designed to evolve with your needs. We encourage communication and collaboration to make sure you're always receiving the right level of care.
Is there a minimum number of hours for home care services?
For all home care services, the only minimum we have is a two-hour daily minimum. If a caregiver is coming to your home, you will be billed for two hours. We do not have a weekly or monthly minimum. All skilled services are dictated by the insurance provider requirements. We work with you to develop a schedule that supports your needs while meeting eligibility and coverage guidelines.
What areas does PHS serve?
We proudly serve the Las Vegas Valley and surrounding Southern Nevada communities including Pahrump. Contact us to confirm availability in your area.
How do I know if my loved one needs in-home care?
Safety is the main concern. Look for changes in physical ability, hygiene, memory, or emotional well-being. If daily activities become difficult or unsafe, in-home care may help maintain quality of life and independence.
Do you work with hospital discharge planners or social workers?
Yes. We often coordinate care transitions with hospitals, rehabilitation centers, and social workers to ensure a smooth handoff to in-home services.
What’s the difference between home health and home care?
"Home health" typically refers to skilled nursing and therapy services by licensed professionals, while "home care" refers to personal assistance with activities of daily living tasks. PHS offers both, depending on client needs.
