What services does PHS provide?
Skilled care refers to clinical medical services delivered by licensed professionals such as registered nurses, physical therapists, occupational therapists and speech therapists. These services typically require a physician’s order and may include dressing changes, wound care, medication administration, IV therapy, and/or education while recovering from a recent hospitalization or surgery.
Non-skilled care, also known as personal care or custodial care, focuses on activities of daily living support – helping with tasks such as transferring, toileting, bathing, dressing, eating, and continence. These services are often provided by caregivers and aides and do not require a physician’s orders.
Do you accept Medicare?

Do you accept Medicaid?
Yes. PHS accepts straight Medicaid services for non-skilled only. PHS does not accept any MCO plans. We accept Type 30 Personal Care Services, Type 48 Frail Elderly Care Services, and Type 58 Physical Disabilities Care Services. Our intake team helps you navigate eligibility and paperwork.
Are you contracted with the VA?
Yes. PHS is an authorized VA Community Care Network Provider for non-skilled home care providing ADL services.
In the near future, PHS will provide skilled nursing and therapy services.
Can I use my long-term care policy to pay for services?
Yes. LTC policies will pay for activities of daily living services. With our advanced knowledge of long-term care policies, our team can help guide patients through the claim process with the insurance carrier.
Who provides my care?
Our care teams include licensed nurses, certified nursing assistants, and trained home care aides. All caregivers are background checked, thoroughly trained, and chosen with your needs and comfort in mind.
Will I always have the same caregiver?
That is the goal. We strive to provide continuity and consistency. The best way to accomplish that goal is to maintain a consistent schedule and to be kind to your caregiver. If a change is ever needed, we will work with you to ensure the new caregiver is a good fit.
Can I adjust my care plan?
Your care plan is designed to evolve with your needs. We encourage communication and collaboration to make sure you're always receiving the right level of care.
Is there a minimum number of hours for home care services?
For all home care services, the only minimum we have is a two-hour daily minimum. If a caregiver is coming to your home, you will be billed for two hours. We do not have a weekly or monthly minimum. All skilled services are dictated by the insurance provider requirements. We work with you to develop a schedule that supports your needs while meeting eligibility and coverage guidelines.

How do I know if my loved one needs in-home care?
Safety is the main concern. Look for changes in physical ability, hygiene, memory, or emotional well-being. If daily activities become difficult or unsafe, in-home care may help maintain quality of life and independence.
Do you work with hospital discharge planners or social workers?
Yes. We often coordinate care transitions with hospitals, rehabilitation centers, and social workers to ensure a smooth handoff to in-home services.
